Transitioning Phase

This is the third of five phases of transitioning out of the ADF. It begins with you formally declaring to Defence that you will transition and have completed and submitted the AC853 form or Defence advises you that you are to be involuntarily separated from the ADF (eg. for medical reasons) and ends at your official transition date. This phase involves actioning the checklist of items Defence requires of you to transition and is managed by your unit and ADF Transitions Centre, Lavarack Barracks for soldiers, sailors and aviators transitioning from service in North Queensland.
